Welcome to FormOrigin!

These Terms of Service ("Terms") govern your use of the FormOrigin platform - the marketing attribution solution that helps businesses track lead sources by storing first-touch attribution data and seamlessly injecting it into website form submissions.

By using FormOrigin, you're entering into a binding agreement with Coscorp Pty Ltd (ABN 72 612 597 494), based in Sydney, New South Wales, Australia. If you don't agree with these Terms, please don't use our services.

About Our Service

FormOrigin offers marketing attribution tools accessible via our website, API, and associated platforms. All plans require payment - we do not offer free accounts.

Our 14-Day "Prove It Works" Money-Back Guarantee

We're confident our marketing attribution tool will deliver value. Try it for 14 days from your purchase date. If you've installed it, configured it correctly, and processed real leads through your forms, but still feel it's not the right fit, we'll refund your annual fee.

Note: To claim a refund, you must provide evidence of proper installation and active use with lead data within the 14-day period. This guarantee is available for annual plans only.

Your Account

When you register with FormOrigin:

  • You must provide accurate, complete information and keep it updated
  • You're responsible for maintaining the security of your account credentials
  • You agree not to share your account with unauthorized users
  • If your email domain is controlled by an organization that establishes a relationship with us, your account may become part of that organization's managed accounts

Subscription Plans & Payment

  • We offer monthly and annual subscription plans
  • Agency plans allow integration with multiple websites based on tier level
  • Single-site users are required to purchase annual plans
  • All payments are processed in USD via credit card
  • Your subscription automatically renews until you cancel it
  • To avoid being charged for the next period, you must cancel at least 3 business days before your renewal date
  • We reserve the right to change our pricing with advance notice by email

Cancellation

  • You may cancel your subscription through the account settings within the app
  • Upon cancellation, your service will continue until the end of your current billing period
  • No refunds are provided for partial billing periods
  • After termination, we may delete your data at our discretion

Your Rights & Restrictions

You may:

  • Use FormOrigin for its intended purpose of tracking marketing attribution
  • Access features included in your specific subscription tier
  • Cancel your subscription at any time (subject to our cancellation policy)

You may not:

  • Use FormOrigin to harm others or for any illegal purpose
  • Attempt to reverse engineer our service
  • Resell or redistribute FormOrigin
  • Generate excessive traffic or automated requests that violate our fair use policy
  • Use the service to distribute malware or harmful content

Fair Use Policy

FormOrigin is designed for regular website traffic from legitimate business activities. We do not permit:

  • Bot traffic or automated scraping
  • Deliberate overloading of our systems
  • Any activity that negatively impacts service performance for other customers

We reserve the right to limit or suspend service to accounts that violate this policy.

Privacy & Data

  • Our collection and handling of information is governed by our Privacy Policy at formorigin.io/privacy-policy
  • You grant us permission to use data you provide to deliver and improve our services
  • We treat your billing information and account data as confidential

Communications

By using FormOrigin, you consent to receive service-related communications and occasional marketing messages. Important service notices will be delivered via email.

Intellectual Property

  • FormOrigin and all related materials are owned by Coscorp Pty Ltd
  • You receive a limited license to use our service based on your subscription tier
  • Any feedback or suggestions you provide become our property
  • We respect others' intellectual property and expect you to do the same

Marketing

By using FormOrigin, you grant us permission to use your company name and logo for marketing purposes. If you wish to opt out, please contact us through the feedback form in the app.

Service Updates & Availability

  • We may update, modify, or enhance FormOrigin at any time
  • We strive for high availability but do not guarantee uninterrupted service
  • We reserve the right to discontinue features with reasonable notice

Limitation of Liability

To the extent permitted by law:

  • We provide FormOrigin "as is" without warranties of any kind
  • Our liability is limited to the greater of: (a) the amount you paid us in the previous 12 months, or (b) $100 USD
  • We are not liable for any indirect, special, or consequential damages

Legal Framework

  • These Terms are governed by the laws of New South Wales, Australia
  • Any disputes must be resolved in the courts of New South Wales
  • Before filing a claim, we both agree to attempt informal resolution
  • If any part of these Terms is found unenforceable, the rest remains valid

Changes to These Terms

We may update these Terms occasionally. We'll notify you of significant changes by email before they take effect. Your continued use of FormOrigin after changes indicates your acceptance.
